Before your first visit

A little preparation goes a long way.

Completing your paperwork ahead of time gives your therapist a head start on getting to know your child, and means more of your first visit can be spent on what matters most.

  • Print and complete your forms. You’ll find every new-patient form on our forms page.
  • Check your insurance. We’ll help you verify coverage and find out whether a referral or pre-authorization is required.

Common questions

Questions families often ask

How do I know which therapy my child needs?

You don’t need to know before you call. Tell us what you’re noticing, and we’ll help you decide which service makes sense as a starting point. Because our speech, occupational and physical therapists work side by side, it’s easy to add or adjust services as we get to know your child.

What ages do you work with?

We provide services to children of all ages, with a wide range of needs and diagnoses.

Do I need a referral?

It depends on your insurance. When you call, we’ll guide you through verification and help you find out whether a referral or pre-authorization is required.

Can my child see more than one therapist?

Absolutely. Many of our patients receive a combination of speech, feeding, occupational and physical therapy. Let our front desk know, and we’ll do our best to schedule appointments.

What insurance do you accept?

We’re currently in network with Anthem, Medical Mutual of Ohio and Tricare. Visit our insurance page for details and alternative funding resources.

When are you open?

We see children by appointment only, Monday – Thursday, 8 AM – 6 PM. The best way to reach us is by phone at (937) 427-1919.
